COMPLETE POKEMON RESET

Please read carefully!

This form is used to completely reset a Pokemon's levels to 0.
This can only be done by the person who owns the Pokemon, and is separate from resets required by design changes resulting from trades.
All learned/bred moves will be reset to static moves.
A Pokemon evolved under the members' care will be reset to its base stage or its stage received at. A Pokemon whose trait was changed by questlines under this person (immune, shadow+, etc) will be reset to its original state at obtainment.
It is a moderator's responsibility to check all of the provided information to make sure the Pokemon is reset to a correct state.
